Consent forms

 

In order for us to be able to use the actors in our film opening, we needed their consent. We had them sign a consent form which mean they would agree to a number of different things.

For example it would allow us to;

 

• Record their performance.
• Adapt/Edit their performance in what ever way we see fit.
• Put the final piece on our blogger page and YouTube channel.

They also had to put whether or not they wanted to be credited in the piece.  

 

 

 

 

Here you can see that both Ben and Dom have signed the consent forms and both would like to be credited in the in the work. This is a legal requirement since this video will go up on YouTube and therefore be available to view online.
